The PACE model is centered on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in their community whenever possible.

Seniors, their family members, and caregivers face many issues, including transportation to appointments, management of medications, coordination of medical care from different specialists, lack of social interaction, and ability to stay alone at home. On Lok PACE participants receive in home care services and transportation to a On Lok PACE center for primary medical care, social and recreational activities, and other senior care services.

POSITION SUMMARY: Under the direction of the Director of Clinical Services, the Nurse Manager is responsible for providing leadership and direction to the nursing department (clinic and home care departments) and integrating this department's leadership and functions with those of the other departments to fulfill On Lok Senior Health Services' mission.


DUTIES / RESPONSIBILITIES:

CLINIC & HOME CARE NURSING SERVICES & QUALITY ASSURANCE

  • On an ongoing basis, plan, implement and evaluate clinic nursing and home care service delivery operations in the clinic and home based settings. Assure quality standards are met or exceeded. Ensure effective monitoring of key performance indicators and standards. Incorporate high satisfaction rates of participants, families and employees as a measure in monitoring staff performance.
  • Develop and achieve written goals and objectives for the department and integrate with program services. Be actively involved in nursing and program operations to ensure that staff achieves stated goals and operational targets. Appropriately delegate responsibilities and functions to those directly supervised, allowing them the latitude to identify the ways and means of accomplishing goals.
  • Provide for continuity of care by recruiting, developing and motivating employees and by developing systems to ensure this is accomplished across all teams. Provide direction, mentoring, guidance and support to the nursing and home care staff to ensure quality nursing service delivery. Maintain high employee morale. Encourage professional and personal development and use regular ongoing training sessions to improve quality, productivity and minimize employee turnover.
  • Understand regulations and promote and ensure compliance with all Medicare, Medicaid and state facility and health plan licensing standards applicable to On Lok. Respond to deficiencies of site audits when necessary, and implement and monitor corrective action plans.
  • Standardize policies and procedures.

SERVICE DEVELOPMENT

  • Evaluate operational systems and fine tune or create new procedures, bringing an analytical orientation to daily operations. Implement a best practice approach and assure staff are trained to provide services effectively and efficiently.
  • Anticipate and respond creatively to new service demands in the marketplace (consumer preference or regulatory changes) for the nursing and home care departments. Provide leadership to facilitate change management processes. Serve as a sounding board for new ideas and approaches.
  • Align departmental goals with organizational goals and support necessary operational changes.

STAFF DEVELOPMENT

  • Create and support a learning environment.
  • Provide support and mentoring to staff in their development of personal and professional goals.
  • Collaborate with Program Managers/ Director of Clinical Services to provide oversight, development and monitoring of staff's performance within their discipline and on the team. Ensure staff competencies and training goals are met.

MANAGEMENT TEAM

  • The Nursing Manager provides leadership to the nursing department (clinic and home care depts.). Under the direction of the Director of Clinical Services, the Nursing Manager provides a nursing perspective to the On Lok Senior Health Services program supervisors' team.
  • Works with other members of the OLSHS program supervisors' team to coordinate process improvement initiatives.
  • Participates in long-range planning, special projects and provides a nursing perspective to work plans. In conjunction with the OLSHS program supervisors' team, initiates, develops or reviews nursing policies related to new or expansion of existing programs.
  • Participates in the preparation of annual budgets for On Lok Senior Health Services for the nursing and home care departments. Effectively manage nursing resources.
  • Works with colleagues and staff to develop consensus and negotiates solutions consistent with On Lok's mission, business and operational goals. Actively promotes a learning organization by encouraging open communication.
  • Works with other managers and administrative staff to promote positive relationships.
  • Other related duties as assigned.

 

QUALIFICATIONS (knowledge, skills, abilities):

  • Associate's or Bachelor's Degree in Nursing; Master's in prepared health related field preferred.
  • Active RN License
  • 3 years management experience in clinic or home care setting.
  • 1 year experience working with the elderly or frail population.
  • Proven track record for management and leadership of nursing staff.
  • Demonstrated ability to maintain an upbeat, positive approach to managing, reinforcing positive behaviors and creating a highly effective team.
  • Skilled facilitator and problem solver with excellent communication and interpersonal skills.
  • Computer literate in Microsoft Office.

Your final compensation offer will be determined based on factors such as skills, experience, education, and licenses/certifications. The pay range is expressed as an hourly rate regardless of FLSA status. Exempt positions are paid salaried and are not subject to overtime.
